Display HCC Weights in Epic

A number of our providers have asked that the weights of individual HCC diagnoses be displayed in Epic. Any thoughts on this would be appreciated.

    Our Epic shows just the Actual score, Potential and GAP score to providers.

    I have an additional question which is sort of in this same thread. Does anyone know if within an HCC category such as HCC 18 - if the patient has more than one diabetic complication such as E11.21 and E11.3211, would the RAF weight value and reimbursement be higher than just HCC 18?
