Ambulatory/Outpatient CDI Measures

Hello,


I am looking to see how others are tracking and measuring the success of their ambulatory CDI program. We specifically work in the primary care practices and have nurses perform prospective chart reviews and coders perform prospective and retrospective reviews. We are looking for productivity metrics and ways other systems are tracking the success of their program. Is it through HCC capture rates? Individual physician RAF scores?
