Hypotension and dehydration
Any thoughts on how to sequence hypotension and dehydration?
The patient was treated with IV fluids, adjusted medication; off lasix, and decreased metoprolol.

Hard to answer your question without any clinical information. What does the severity of signs and symptoms tell you as the patient was admitted to the hospital as evidenced in the physican's clinical documentation. What picture is painted by the nursing clinical documentaton. What diagnosis based upon the clinical picture and management of the patient best represents the reasonableness and necessity for admission. Reasonable and necessary and supporting documentation of the same should the overarching principle for principle diagnosis selection.
