Escalating to ALJ

Hello!

My facility currently has no process to escalate denials to ALJ and I have never done so. I've looked through CMS' website and see that there is a form to fill out (OMHA-100). Are there any other steps I must take to escalate our denials to ALJ? We've exhausted our appeal rights for the MA payer.
