Do you query for time?

I have a couple of questions regarding the documentation of time for telephone visits. If a provider has done a telehealth encounter using only the telephone and they forgot to document the amount of time, do you query the provider? I'm not sure that the provider would actually remember how much time he/she spent. If the provider adds an addendum to their progress note, do you submit a corrected claim if it was billed?

What about other visit types that require time to be documented?
