Acceptable MEAT

The providers I work with are definitely minimalists so I am trying to squeeze anything out of the note that might support MEAT. If a provider codes carotid arterial disease and in the plan says "follow up with cardiology as planned", does this count?

I have another provider who lists the patient's specialists and indicates the specialty but I don't think this is really enough. Do you agree?

I appreciate any help with this. I have no one in my organization who can answer questions.

    I would say neither scenario you mentioned would qualify for reporting as a secondary diagnosis.

    I ask my providers to always ensure their written diagnoses have plenty of STEAM (specificity, treatment, evaluation, assessment, and monitoring). This way the acronym is also in order of documentation importance, in my opinion.
