outpatient CDS

So I did not get but one response when I asked if anyone is working on the outpatient side of CDS... I am really looking for someways of getting started on improvements for outpatient documentation and reimbursements.

    It was a new concept to me at conference. We just started our program in Aug 2011 so even thinking of going to the Outpatient is a couple years away.
  • The coders here are doing the outpatient reviews and queries at my facility. I'm available for consult if they have clinical questions, but my focus is on inpatient. We do use the same tracking tool and same query forms.

    We are currently rolling out a CDI opportunity in our ED. It is starting with obtaining RN documentation for infusions, injections and hydrations. I am finishing up training this week and our go live is May 2nd.

    We do not do any out patient reviews, and have not been asked to collaborate with the out patient coders.

    Yes I see there was a presentation at the conference, but I cannot get the slideshow to open on the ACDIS website....

    So I am assuming you are looking at stop/start times, and diagnosis
    associated with dehydration to make sure you are capturing....any other
    info would be greatly appreciated.

    Yes, those you mentioned and the following:
    - Route (can't charge without a route).
    - If more than 1 IV started. If so if all sites are doc'd
    - If RN and MD medication documentation reconcile
    - If infusions/hydration are documented as still running or stopped at the time of an admission (if the patient is admitted)

    From what we've seen so far, hydrations are going to be our biggest area of improvement.

    Two things to try -- there are several of the ppts that I can not open on my older desktop that is running MS Office 2003.

    PPTs, etc. that are done in much more recent versions of office won't open under older versions. I think the cut is you need somewhere around 2007 or newer. If you can not access a more recent MS Office and then save as an older version -- download Open Office (an open source office suite) that is generally compatible with the newest MS suite.

    I don't review the outpatient charts for reimbursement, however I have gone over documentation that will support the outpatient billing for the physicians. I do review the outpatient claims for RACs. If we find an issue, we correct it.
