Uncontrolled DM

Does anyone have any definitions as to classification of "uncontrolled" diabetes. I have seen numerous guidelines when it comes to CDI but nothing from a medical body.
I have been trying to find something from the ADA, but I haven't had any luck.

I am creating a tip flyer for Dr's and I wanted to provide some criteria I could site as an example.

There is an article callled "Diabetes Classification Integral to Hospital Quality Measurement" ACDIS Oct. 6, 2008 which states:

"The criteria for uncontrolled diabetes is individualized by the treating physician; however a HgbA1C greater than 7% or multiple. Blood Glucose greater than 250 mg.dl usually indicates uncontrolled diabetes".

I have this taped into my CDI Pocket Guide.
