Modified Rankin Scale

Our neurology groups use the Modified Rankin Scale (docuemented only by clinician), which measures the degree of disability or dependence in the daily activities of people who have suffered a stroke. Looking at quality indicators for length of stay and mortality--"debilities" includes wheelchair bound and bedbound.

I am curious if any hospitals code these debilities based on the modified rankin scale?
