I'm wanting to apply a Z91.130 code to specify the unintentional underdosing due to patient's age related debility, medication noncompliance, on a CHF patient who is being readmitted for decompensated CHF less than 30 days after discharge for the same diagnosis. The ED note states that the patient did not take his medications and for this same reason, was admitted for CHF on the previous admission. How would you compile a compliant query to get the noncompliant code added?
