Simple/Moderate Laceration Repairs

Hello,

If a provider documents in the Op Note - Simple Laceration Repair, can you take that to Repair of the Skin.

Also, if the provider documents - Intermediate Laceration Repair, can you take that to Repair of the Subcutaneous Tissue & Fascia?

Or do you need to query to clarify the depth?

Procedure: Intermediate Laceration of left wrist repair. 

Procedure in Detail: Intermediate Laceration of left wrist repair. 4.0 cm in length. Wound was copiously irrigated with betadine, normal saline, andhydrogen peroxide mixture.
