AKI - 0.3mg/dL criteria
Hello,
A patient has creatinines that are within our facilities range of normal (0.7-1.30mg/dL).
The values are as follows: 1.17, 0.92, 0.87, 0.79, 0.94, 1.21, 1.15.
0.79 + 0.3 = 1.09
Even though these values are within normal limits - would you still query for AKI?

What are the time frames between those creatines for one thing?
The kdigo criteria regarding 0.3 is a 0.3 increase within 48 hours from a measured value. If exceeds 48 hours, that criteria should not be used.
Prospectively and retrospectively was confusing to me for a long time ( so I think I just ignored it :)), but if I understand it correctly in regards to the above scenerio.
I'd take the 1.17 and divide by 1.5 which would need to drop to 0.78 ( so close!) and may not qualify.
(1.5x you can use prospectively or retrospectively)
to use the 0,3 it would have to go up (prospectively). so from arrival, it would need to increase to 1.47.
As far as "values are within normal", like most things it will come down to provider opinion. Some may feel it needs to be outside of your labs normal range and others may feel a significant change is relevant to a patient with a very low baseline.
