PSI 11 without Acute Post Op Respiratory Failure, or even Resp. Failure at all

Case of lady taken to OR for cardiac procedure, left on mechanical ventilation because the decision was made to leave the patient's chest open and return to the OR in 1-2 days. Pt was taken back to the OR, chest closed, and extubated at the 96-hour mark. The pt hit a PSI 11, and there was never an Acute Respiratory Failure documented. Is there any coding guidance around a situation such as this? Thoughts? Comments?
