Coding stroke recrudescence

I have been noting the term "stroke recrudescence" in charts here and there. When patient has an old cva per imaging but nothing acute but came in for acute symptoms "CODE STROKE".


The chart reads "Another possibility would be stroke recrudescence due to hypertensive emergency or AKI for which the recommendation would be to avoid or correct these states".


I am trying to sequence this and having difficulty. Is anyone else seeing this terminology and how are you coding it?
