Do you clarify for every midline shift?

Patient was a ground level fall with had a right SDH, no midline shift. Repeat Head CT the next day showed no significant change in SDH but a 3 mm leftward midline shift. Neurosurgery states this is a minimal shift and recommends to f/u with repeat CT in 2 weeks. Started back on SQ Heparin. No IV steroids, diuretics, hypertonic solution. Patient was initially in ICU but downgraded to progressive care within 24 hours. Do all midline shifts require a query for brain compression?
