I'm looking for a good clarification template/wording for "history of". Can someone share with me how you word your clarification? Thank you!

Depends on your situation, I would develop a query asking to confirm, i.e.,

Please clarify XXX as noted in your h & p of 01/25/24

XXXX

[ ] Confirmed as a current diagnosis

[ ] Resolved, no longer present

[ ] Other, please specify______________

[ ] Decline query, please explain____________
