Retained Surgical Items

I am seeking feedback on the reporting of retained surgical items intentionally left in a patient due to the patient risk of removal.  The UI Health Care coding auditing team is finding inconsistent guidance on this topic.

How are these situations handled at your facility? Do you assign dx T81509A in these situations?
