Hyperlactatemia

We get a lot of physicians who document "Elevated lactate level". It was discussed in our recent quarterly CDI meeting that we should be sending a query for "hyperlactatemia" anytime the lactate is above 2 but less than 4. I cannot find a whole lot of information on this diagnosis and what the risk factors are. Say a patient came in with a diabetic foot ulcer with an initial lactate of 2.1 with a repeat of 1.6. Is this hyperlactatemia?
