Can coders code off the physical assessment?

As a CDI, I am wondering if coders can code from the physical assessment? The physicians note, in the physical Assessment portion states, “patient is obese” or “patient obese in appearance.” I am being told to query for things like cachexia, fraility. One stated appears frail, would I query because the word appears? Thanks!!
