Can we code "combined" CHF, or needs it spelled out with "systolic and diastolic"?
Hi everyone,
I tried to search in Coding Clinic but I am unable to find answers to my question. Let's say it is an acute CHF and echo supports systolic and diastolic CHF. Can we code I50.41 acute combined systolic & diastolic CHF for the documentation "acute COMBINED CHF?" For the type "combined", do we need it to be spelled out as "acute combined systolic and diastolic CHF" or "acute HFrEF and HFpEF" or anything similar to those?
Thank you!

I think you will get varied answers, but working as an auditor I have seen this coded as combined when both are mentioned. However, saying this we all know that different insurances will appeal diagnoses.
