Re: Admission Consultants
We use them. I think our Resource Management Dept. likes them. I prefer an aggressive Preadmission Department myself. One that screens admissions - has a Social Worker or Discharge Planner/Case Manager who is very familiar with available community resources who may redirect these patients from admission. I think this is why many prefer to defer them for review with this company or companies like them. Plus, they follow-up by providing RAC defense.
My concern is that they often approve admissions - which in the days when I performed UM - I would have sought an Observation admission.
It has greatly cut down on the need for Observations admissions, however the Diagnoses usually result in low weight DRG's. (We talked about them in another thread here, I believe. Not by name but by what they do.
As a CDS, the resulting Diagnosis from many of their approved admissions have been...disappointing. Syncope, is syncope, is syncope... In many cases it doesn't matter how specific your PDx becomes (Dehydration, Orthostatic Hypotension, Iatrogenic Hypotension, Hypoglycemia...) it will still result in a low-weighted DRG. Its then that I notice these admissions often have an EHR letter filed in the chart. Just sayin'.
Watch your CMI for a while. Unfortunately, Observation patients are included in CMI so it's difficult to say which is worse - observation status vs. low-weight DRG admission.
