Cerner Millineum

Is there anyone that has a fully electronic record with Cerner Millineum? If so, how are you leaving clarifications (queries) for your physicians?

    Martha Jefferson Hospital has Cerner and we are still using paper
    queries, then scanning them after discharge to have on file if audited.

    We have created a power note brief clarification template that then can be sent directly into the physicians inbox to agree or disagree with. We are trying to figure out a way that the physician can open this up and then have access to directly document. Has anyone figured that out?

  • How are you handling the queries as pertains to complying with the AHIMA brief, e.g., no yes/no questions (except POA) and offering multiple choice options?

    I think this will be a big issue with the RAC requesting queries so I'm hoping you have the solution!

    Our pre-formatted queries from Navigant have several options, as well as a "check here if unable to determine" box for the providers.
    We also have a "blank" documentation clarification query that the CDS staff write in free text, giving examples of possible dx and the same unable to determine option.

    And do you have problems with physicians reading their inbox clarifications(not looking at them)?

    We leave the queries on the charts while the patients are in house. If not answered the Lead Coder sends it back thru their IN-BOX, but we have to type it again and then place the deficiency in their Physician Def piece within Cerner. It is alot of work if they miss it on the floor.

  • We are also moving to Cerner - we are only in the infancy stages though. We are going to start with MD orders and nursing documentation - probably won't go live for about 9-12 months.
    When you leave the concurrent queries are they electronic or on paper. I can't seem to get a straight answer from anyone at my end regarding Cerner's ability to handle electronic queries while patient is still in house or if it can handle the query templates that we have developed.
