Volume Contraction

Does anyone have any issues or suggestions relating to the use of physician documentation of "Volume Contraction"? We have a physician who is an outlier for query response and just keeps writing "volume contraction" with no relationship to any other associated diagnosis. We cannot make an assumption to "hypovolemia", or can we? Does anyone have a query related to this, or suggestions for responses from a query!
