death tracking

Our coding manager and I have been reviewing all in-hospital deaths since april. We check to make sure that the documentation is complete and the coders are picking up all significant diagnoses. When we started, we began collecting data on basic pt info along with our impact (increased SOI/ROM, DRG changes, etc). Later we were asked to also note if the patient was seeing palliative care and whether they were moved to "comfort care" prior to death.

Now, we have been tasked with working with quality to enter this information back into our data analysis system. Administration wants to be able to access death data based on Pdx, DRG, MD, etc.
We know for sure we will be entering;

Cause of death
type of admit (transfer/direct)
days in ICU
Code status
Palliative care
SOI/ROM
Age
DRG
dates
charges
MD


Is anyone else involved in death tracking? what kinds of data are you tracking?

I'm just trying to make sure I am capturing all the data that people want.

    We've been collecting this info for more than a year for our core
    measures teams and are now doing pre-billing review of all mortalities
    as well.
    We run a monthly report from our 3M data collection system. All of
    these data elements are built into the system. Additional data points
    for consideration include:

    Present on Admission Indicator
    Admit Dx
    Nursing Unit
    Last visit date
