staff schedules

Does snyone work a 4 day/10 hr day work week? If so, how is it working out and does all or just some of the staff work that schedule?

    Look in:
ACDIS Journal July 1 2010, Case Study: Alternative staff schedules offer CDI opportunities

    and

Forms & Tools library, ACDIS - Policies, Procedures, Regulations, and Job Descriptions, Sample CDI flexible schedule: http://www.hcpro.com/content/254985.xls

Don't have experience personally, but found those interesting.

  • Hi Kelley,
We worked 10 hour shifts for a while in our program. We worked M-F as a group. Wednesday was our usual day for meetings so everyone worked on Wed. I went to 8 hour shifts for personal reasons and one other nurse followed shortly after. Our other nurse resigned and our Director then made the available positions 8 hour shifts (she found it easier to have consistent staffing numbers when we all work 8 hour shifts). My weekly productivity numbers didn't change when I went to 8 hour shifts. I do miss the extra day off during the week but I know at times I would struggle to keep my attention focused during my last two hours of reading charts when I did 10 hour shifts.

thank you. I too fear the brain fatigue of the last two hours. did you see any benefits besides the one day off? my staff really want to do this but I have to weigh the pro's and con's.

  • I believe the 10 hour shifts to be more attractive for RN's looking for a non-patient care position. Most of our CDS have come from bedside nursing and I know of at least a couple who didn't apply as they did not want to work 5 days/week. I do believe the biggest pro is that it can be a staff satisfier. You are also available more hours/day to communicate with MD's. One of our downfalls with our staffing was that we weren't scheduled appropriately; there were two days a week when there was only 1 out of the 3 of us here. If scheduled appropriately I don't think it would have been an issue.

we have 5 cdi's - 4 on 40 hr , 1 on 32 for a 423 bed hospital which usually is at most at 410 but also can drop to 295 or so. Average is 375.

thank you Cindy. I agree with you about the 4 days appealing to more candidates. that is one reason I am asking about this. I finally have an open position. It is posted on this site.
  • What payors do the CDI staff look at?

well the doc spec only portion of the job looks at all DRG Payors. The other aspects include review all possible payors with admitting diagnoses of chest pain, sob, etc. that could code to AMI or CHF for core measures and any with dizzy, weakness, and the like for medical necessity.

I have worked 4/10's for 2 years now. There are 2 other CDI's and they both have continued to work 8 hr shifts. I have not seen any real change in my stats and the satisfaction of having a 3 day weekend is very satisfying.
