Therapeutic Interchange
We are having problems passing CHF Discharge Medications because the physician is dictating the home med in the discharge summary and also checking to continue the therapeutic interchange on the Discharge Medication Reconciliation. Since only the meds listed on the reconciliation are printed on the patient's discharge instructions, we get a mismatch if the generics are not equal. How does everyone handle this?
