Query Response Rate

Out of curiosity what would you say your query response rate is?
Also are your queries part of the permanent record?
Wondering if there is a difference in the response rate between queries that are or are not part of the permanent record.

  • Ours is around 80%. Our queries are a part of the permanent record. Our problem is that the hospital has not designated queries as a chart deficiency and queries that were unanswered prior to discharged are not routinely followed up on.

  • Our response rate is approximately 95%.
    Since going live with our EHR this past May, our queries are a permanent part of the medical record. We create a deficiency for each of our queries which goes into the attending physician's electronic in-basket. I believe this has had a positive impact on our response rate.

    Hi Greta,



    Our program is still a one man show- me. We started in 4/11 with queries
    which were not a part of the medical record and requested additional
    documentation in the progress notes, but we quickly changed to making
    them a part of the permanent record. We recently made queries a
    deficiency, and the response rate did go up significantly. The
    percentage of queries answered by the last day of the month was up to
    91% for January.

    We are actively looking for an analyst for our CDI dept., so one day I
    will have retrospective data. In the mean time I collect as much data as
    I can, but primarily focus on submitting queries and following up on
    timely replies.
