sp cath

Can anyone tell me if they generally query for UTI due to SP cath. We normally do for indwelling, but do you for an SP cath?

Thanks

The insertion of a suprapubic catheter codes to ‘percutaneous cystostomy’ (57.17). Therefore should the complication of such code to complication of cystostomy? If so, the code for infection due to suprapubic catheter (cystostomy) is 596.81 per CC 4th Quarter 2011. Please let me know if I am looking at this incorrectly!

    · Complication,

    o cystostomy 596.83

    § infection 596.81

    § mechanical 596.82

    § specified complication NEC 596.83

    Cystostomy complications

Coding Clinic, Fourth Quarter 2011 Pages: 128-129 Effective with discharges: October 1, 2011

    Question:

What is the code assignment for an admission for urinary tract infection due to suprapubic cystostomy?

    Answer:

Assign code 596.81, Infection of cystostomy, as principal diagnosis. Assign code 599.0, Urinary tract infection, site not specified, as an additional code for the type of infection. If known, use an additional code to identify the organism.

  • I believe that coding clinic is referring to infection due to the actual cystostomy, not infection due to the SP catheter itself.

This is from an AHIMA presentation:

A patient is admitted to the hospital
    with a urinary tract infection
    secondary to their suprapubic
    catheter.

􀂊 Assign the principal diagnosis code of
    996.64, Infection and inflammatory
    reaction due to indwelling urinary
    catheter.
􀂊 Assign 599.0, Urinary tract infection
    as a secondary diagnosis code.

I am forwarding this response from 3M Nosology regarding UTI due to suprapubic catheter. Prior to 10/01/2011, 996.64 was the correct code. However new codes were established to specifically address cystostomy complications. CC, 4th Qtr, pp 128-129 addresses this (see earlier email). Was AHIMA’s direction to use 996.64 prior to the 2011 date?

The 3M Nosology team recommends the assignment of diagnosis code 596.81 for an infected suprapubic catheter in a cystostomy; assign 599.0 for the urinary tract infection and an additional code to identify the organism, if known.

The suprapubic catheter is not an indwelling or urethral catheter, therefore code 996.64 would not be appropriate. The new code (effective 10/1/11) for infection of cystostomy was discussed in the AHA Coding Clinic® for ICD-9-CM, Fourth Quarter 2011, pages 128-129:

    · when a person has problems with urination, a suprapubic cystostomy may be performed to create a connection between the bladder and the outside of the body. A catheter is placed through the abdominal wall above the pubic bone and into the opening to drain urine from the bladder. This procedure is usually performed when a catheter cannot be advanced into the bladder through the urethra. Urine is drained from the bladder and into a collection bag. The catheter is kept in place until scar tissue forms a tract between the bladder and exterior abdomen. Once the tract forms, the catheter can be removed periodically to prevent infection. When the cystostomy is no longer necessary, the catheter is removed permanently to allow closure of the tract. In addition to infection, complications of cystostomy include leakage of urine, peristomal breakdown of skin and failure of the tract to close.
