What depth would you code this wound to? Need objective opinion.
Wound #1 Open. Original cause footwear injury. Location: Left Calcaneous. Measures: 0.9 cm length x 0.4 cm width x 0.3 cm depth: 0.28cm^2 area and 0.085cm^3 volume. Limited to skin breakdown. No tunneling or undermining noted. Medium amount of serosanguineous drainage. Margin well defined not attached to wound base. No granulation within wound bed. Large (67-100%) amount of necrotic tissue within the wound bed including adherent slough. Perwound skin: dry/scaly.
Wound # 1 Diabetic wound/ulcer of lower extremity L Calcaneous. There was a skin/subcutaneous tissue debridement with total area of 1.2 sq cm performed with curette to remove viable and non-viable tissue/material including exudae, fibrin/slough and subcutaneous. Post debridement measurements: 0.9 cm length x 0.3 cm width x 0.4 cm depth: 0.085cm^3 volume.
