Outpatient HCC Question

Hello all,

I am currently researching for an outpatient CDI program for a physician practice. I have a question: the HCC model applies to Medicare Advantage and all ACO's right? Well who participates in ACO's? I am not finding any information on this. We have a very small percentage of Medicare advantage. High percentage of straight medicare. Can anyone help please?

There are several HCC models- CMS HCCS and HHS HCCs are two. CMS HCCs pertain to medicare advantage plans. I am not about all ACOs.

For ACO they use the HHS HCC as Medicaid population and Medicare advantage uses CMD HCCS.
