CM, CHF, CKD, HTN

What is your opinion on PDx assignment for a patient coming in with newly diagnosed Cardiomyopathy causing CHF, associated known ckd, htn?  I want to use the CM as PDx, but think I have to use the combo code for Htn Hrt and CKD.

Thoughts?  Thank-you.

  • Unless the record documents some other cause, the code for hypertension WITH CHF and CKD is the PDX.   (I13.0) - followed by code for the type of CHF,  HTN and CM.

See index and tabular as well as official coding guidelines.

For a patient in acute CHF, the CM would not be used as the PDX.
