Hi,

If provider documents excisional debridement down to bone, is the procedure code then include bone or level above.

    Yes it will coded as excision of the bone(specify name of the bone excised)

    We teach the provider should state " down to AND INCLUDING" so that is very clear as to how deep they excised. So for example "down to and including the bone" leaves no room for misunderstanding.
