DNR order changed to Comfort Care

Good morning,

May I solicit your feedback on a question that came up in a CDI/Coding mortality discussion at our facility?

A patient with DNR on admission (noted in H&P, Order, notes), rescinds their DNR status to “Full code” and passes away.  QUESTION: Is the DNR coded since it was rescinded?  Thus negating the Mortality factor the DNR would have had on the case?

Per ICD-10-CM Official Coding Guidelines, page 24,  the code Z66  (do not resuscitate) may be used when it is documented by the provider that a patient is on do not resuscitate status at any time during the stay.

  • Kathryn, I would suggest confirming with a Coding clinic Question but i think if the DNR was rescinded it cannot be coded.
