We manually track in an excel spreadsheet (can be found under resources here on ACDIS that I made when we first implanted our program). We document each patient and the corresponding diagnosis with the HCC that we have contributed to the patients record from our prospective or retrospective review. We then go into the providers note after the provider visit to see if the provider assessed and documented the condition and then if it was coded accurately to close the loop. Each month we total the RAF score for each practice and multiply that by the amount CMS has given us as the multiplier for our facility for the shared savings to come up with a dollar amount impact that CDI has made. Tedious for sure but there isn't any tracking software out there for outpatient as of yet. We also audit our coders which we track in the same manner.
